What is Consumer Health Data?

Under the Washington My Health My Data Act (MHMDA), "consumer health data" includes data that identifies a consumer's past, present, or future physical or mental health status. For MindMentor, this includes: • Mood entries and emotional state tracking • Journal entries related to mental wellbeing • Sleep logs and patterns • Self-assessment results (PHQ-9, GAD-7) • Safety plan information • Gratitude entries • Messages sent to the AI wellness companion

How We Collect Health Data

MindMentor collects consumer health data in the following ways: • Directly from you: When you enter mood entries, write journal entries, log sleep, complete assessments, or send messages to the AI companion • Generated by the app: Mental health scores, streak counts, and wellness summaries derived from your inputs We do NOT collect health data from third-party sources, data brokers, or by tracking your behavior outside the app.

Purpose of Collection

We collect and use consumer health data solely for: • Providing the core app functionality (mood tracking, journaling, AI companion responses) • Generating personalized wellness insights and daily tips • Displaying your progress over time (charts, streaks) • Processing AI companion messages through Azure OpenAI We do NOT use your health data for: • Advertising or marketing • Selling to third parties • Building consumer profiles for sale • Discriminatory purposes

Data Storage Location

The majority of your consumer health data is stored locally on your device: • Mood entries — device only (AsyncStorage) • Journal entries — device only • Sleep logs — device only • Assessment results — device only • Safety plans — device only • Gratitude entries — device only • Onboarding responses — device only Data transmitted to servers: • AI chat messages — processed by Azure OpenAI (not stored permanently) • Authentication tokens — Azure Entra External ID • Subscription status — RevenueCat

Sharing of Health Data

We share consumer health data only with the following categories of third parties, and only as necessary to provide the service: • Azure OpenAI (Microsoft) — processes AI chat messages to generate responses • Azure Communication Services (Microsoft) — facilitates chat message delivery • RevenueCat — manages subscription status (does not receive health data) We do NOT sell, rent, or trade your consumer health data. We do NOT share health data with data brokers, advertisers, or any entity for purposes unrelated to providing the MindMentor service.

Your Rights Under MHMDA

Washington consumers have the following rights regarding their consumer health data: • Right to Know: You may request confirmation of whether we are collecting, sharing, or selling your consumer health data • Right to Access: You may request a list of all third parties with whom we have shared your consumer health data during the prior 12 months • Right to Delete: You may request deletion of your consumer health data (Settings > Privacy > Delete All Data) • Right to Withdraw Consent: You may withdraw consent for collection of consumer health data at any time To exercise these rights, contact us at privacy@mindmentor.live.

Consent

By using MindMentor, you consent to the collection and use of your consumer health data as described in this policy. You may withdraw consent at any time by: • Deleting your data through Settings > Privacy > Delete All Data • Deleting the app from your device • Contacting us at privacy@mindmentor.live Withdrawal of consent will not affect the lawfulness of processing based on consent before its withdrawal.

Geofencing Prohibition

MindMentor does NOT use geofencing technology around any mental health facility, provider, or related location for the purpose of identifying or tracking consumers seeking mental health services. We do not collect location data at any time.
